Treatment of refractory ascites with an automated low flow ascites pump in patients awaiting liver transplantation

چکیده
For patients with liver cirrhosis awaiting transplantation who suffer from refractory ascites (RA), standard treatment consists of repeated large volume paracentesis. The automated low-flow ascites-pump (alfapump) offers an innovative alternative for RA, if TIPSS is contraindicated or ineffective. This study addresses the feasibility alfapump in transplantation. Between 2012 and 2018, listed were treated included this retrospective single centre study. Of 22 alfapump, 14 finally transplanted. Alcohol-related disease was most common aetiology ( n = 11), followed by hepatitis C, B NASH. Mean age at listing 56.3 years 68.2% male. average daily pumped device 1076 ml. During transplant surgery, no alfapump-related complications occurred. removed end procedure eight left place three up to 104 days, whereas had pump prior Overall survival significantly better that transplanted (log-rank p < 0.0001). Five (22.7%) required least one re-intervention. Treatment on waiting list feasible. did not affect explanted surgery.

Treatment of refractory ascites with an automated low‐flow ascites pump in patients with cirrhosis
BACKGROUND Refractory ascites (RA) is a frequent complication of cirrhosis, requiring large volume paracentesis or placement of a transjugular intrahepatic portosystemic shunt (TIPSS). The automated low-flow ascites pump (alfapump, Sequana Medical AG, Zurich, Switzerland) is an innovative treatment option for patients with RA. AIM To assess safety and efficacy of this treatment in patients wi...
متن کاملRefractory ascites after liver transplantation: an analysis of 1058 liver transplant patients at a single center.
A retrospective study of 1058 liver transplant recipients was performed to determine: (i) the incidence, etiology, timing, clinical features and treatment of refractory ascites (RA), (ii) risk factors for RA development, (iii) predictors of RA disappearance, (iv) predictors of survival following RA and (v) the impact of RA on patient survival. Sixty-two patients (5.9%) developed RA and its disa...
متن کاملAscites, refractory ascites and hyponatremia in cirrhosis
Ascites is the most common complication related to cirrhosis and is associated with increased morbidity and mortality. Ascites is a consequence of the loss of compensatory mechanisms to maintain the overall effective arterial blood volume due to worsening splanchnic arterial vasodilation as a result of clinically significant portal hypertension. In order to maintain effective arterial blood vol...
متن کاملSplenic Artery Embolization for Treatment of Refractory Ascites After Liver Transplantation
Post-transplantation refractory ascites is uncommon; however, it can be a serious problem, increasing both morbidity and mortality in patients. Despite scant literature available, splenic artery embolization (SAE) has been shown to be an effective treatment for refractory ascites after cadaveric orthotopic liver transplantation (OLT). We report a successful use of therapeutic SAE for refractory...
متن کاملSplenic artery embolization for the treatment of refractory ascites after liver transplantation.
Refractory ascites (RA) is a challenging complication after orthotopic liver transplantation. Its treatment consists of the removal of the precipitating factors. When the etiology is unknown, supportive treatment can be attempted. In severe cases, transjugular intrahepatic portosystemic shunts, portocaval shunts, and liver retransplantation have been used with marginal results. Recently, spleni...
